We’re excited to meet you and look forward to partnering with you on your healthcare journey. To ensure your first visit goes smoothly, please review the steps below to prepare for your appointment.


STEP 1: Complete Your Intake Paperwork

Before your appointment, please complete your intake paperwork online. Simply select your office location below and fill out the required forms.

If you’re unable to complete the paperwork ahead of time, don’t worry! You can fill it out electronically when you arrive at our office. However, please remember to bring a valid photo ID for verification purposes on the day of your appointment.


STEP 2: What to Bring to Your Appointment

  • Valid photo ID (driver’s license, state ID, or passport)
  • Insurance card (if not uploaded online)
  • Any records from previous healthcare providers
  • Recent radiology reports (X-rays, MRIs, CT scans, etc.)
  • Laboratory test results
  • List of current medications and supplements

STEP 3: What to Wear

Please wear loose, comfortable clothing similar to what you would wear to the gym. This allows for an easier and more thorough examination process. Avoid tight-fitting garments or clothing with complicated fasteners.


STEP 4: Your First Visit

During your initial appointment, your provider will conduct a comprehensive health history review and thorough examination. This is your opportunity to discuss any health concerns, symptoms, or questions you may have. We encourage open communication to ensure we understand your health goals and needs.

Following your examination, your provider will present a detailed Report of Findings that includes:

  • Your diagnosis and what it means
  • Recommended treatment plan
  • Frequency and duration of visits
  • Estimated cost of care
  • Time for your questions and concerns

If additional diagnostic testing is needed, your provider will discuss this with you and provide the necessary referrals. In many cases, if appropriate, your provider will begin treatment during your first visit.
